Our Services

Sierra Pathways currently provides one focused program designed to meet the needs of Transitional Age Youth (TAY) with substance use and co-occurring mental health challenges.

Our services are intentionally streamlined to ensure clarity, consistency, and high-quality care.

Day Treatment Intensive (DTI) program

Sierra Pathways operates a Day Treatment Intensive (DTI) program for Transitional Age Youth (approximately ages 16–25). This program offers structured, evidence-based treatment while allowing participants to remain connected to their families, education, employment, and community.

The DTI is appropriate for young people who need more support than traditional outpatient counseling but do not require residential or inpatient care.

What Our Day Treatment Intensive (DTI) program Includes

Participants enrolled in our Day Treatment Intensive program may receive:

  • Comprehensive assessments and individualized treatment planning

  • Structured group counseling focused on mental health recovery, substance use recovery, and life skills

  • Individual counseling tailored to clinical, developmental, and recovery needs

  • Integrated treatment for mental health and co-occurring substance use conditions

  • Medication-Assisted Treatment (MAT), when clinically indicated, coordinated with qualified prescribing providers

  • Trauma-informed and culturally responsive care

  • Relapse prevention, emotional regulation, and coping skills development

  • Care coordination with families, schools, probation, and community partners when appropriate

All services are delivered in accordance with Medi-Cal Specialty Mental Health Services standards and evidence-based practices.

Who the Program Is For

Our Day Treatment Intensive (DTI) program is designed for Transitional Age Youth who:

  • Are experiencing mental health disorders with or without problematic substance use

  • Have co-occurring mental health and substance use symptoms or diagnoses

  • Are navigating major life transitions related to adulthood, identity, education, housing, or independence

  • Benefit from a structured, supportive, and developmentally informed treatment environment
