About Sierra Pathways
Sierra Pathways is a behavioral health organization dedicated to supporting Transitional Age Youth (TAY) as they navigate the complex challenges of early adulthood, substance use recovery, and mental health wellness. Our mission is to provide compassionate, evidence‑based, and developmentally appropriate treatment that meets young people where they are and supports sustainable, long‑term recovery.
Founded on the belief that healing happens through connection, dignity, and individualized care, Sierra Pathways serves young adults with co‑occurring substance use and mental health disorders, helping them build stability, resilience, and purpose.
Our Program
Beginning February, Sierra Pathways will operate one core program:
Drug Medi‑Cal Intensive Outpatient Program (IOP)
Our Intensive Outpatient Program is designed specifically for Transitional Age Youth (approximately ages 16–25) who are experiencing substance use challenges alongside mental health concerns such as depression, anxiety, trauma, mood disorders, or other co‑occurring conditions.
The IOP provides a structured yet flexible level of care that allows participants to receive comprehensive treatment while remaining connected to their families, education, employment, and community.
Services offered through our Drug Medi‑Cal IOP include:
Comprehensive assessments and individualized treatment planning
Group counseling focused on substance use recovery, mental health, and life skills
Individual counseling tailored to each participant’s clinical and developmental needs
Integrated co‑occurring disorder treatment addressing both substance use and mental health
Trauma‑informed and culturally responsive care
Relapse prevention, emotional regulation, and coping skills development
Care coordination and collaboration with families, schools, probation, and community partners when appropriate
Our program is grounded in evidence‑based practices and informed by a harm‑reduction, recovery‑oriented, and youth‑centered philosophy.
Who We Serve
Sierra Pathways specializes in serving Transitional Age Youth (TAY) who:
Are experiencing substance use disorders or problematic substance use
Have co‑occurring mental health conditions
May be navigating transitions related to adulthood, identity, education, housing, or independence
Benefit from a supportive, developmentally informed treatment environment
We recognize that TAY have unique needs that differ from adolescents and older adults, and our services are intentionally designed to reflect this critical stage of life.
